We look forward to welcoming you to our office! To help make your first visit as smooth and comfortable as possible, please complete all new patient forms before your appointment. You can conveniently upload a valid photo ID and your dental insurance card (if applicable) directly through our digital forms. Please also include a list of any medications you are currently taking.
For the most accurate diagnosis and treatment planning, we take new digital X-rays at our office as part of your initial visit. While we appreciate having your previous dental records for reference, X-rays taken at other offices cannot be accepted in place of our own.
During your first appointment, our team will review your health history, take any necessary digital X-rays, and perform a thorough examination of your teeth, gums, and mouth. We will discuss our findings with you, answer any questions, and work together to create a personalized treatment plan that fits your needs and budget. Our goal is to make every visit thorough, efficient, and comfortable so you can feel confident about your dental care from day one.
